• Applicants can apply in person at Criminal record register of General prosecutor´s office of the
Slovak republic, Kvetná 13, Bratislava, or at any post offices where with IOMO–about 600 in the
Slovak republic
• Applications can be made online (Slovak only) via the General Prosecutor’s Office of the Slovak
Republic (https://esluzby.genpro.gov.sk/ziadost-vo-rt) by applicants who have access rights and
privileges to use the electronic services of the General Prosecutor's Office. To access services, the
applicant must authenticate via their identity card eID card with the appropriate reader.
• Overseas applicants are advised to apply through the Slovakian embassy. Forms are provided by
the offices and embassy, or can be ordered. We have information on the Slovak Embassy in
